Description of service(s) This exercise and education program is for people with heart disease. It helps people recover and get their strength back and reduces their risk of a relapse. 
The program: 
 
Improves physical fitness and the ability to do everyday tasks; 
Improves quality of life and self-confidence; 
Reduces the risk of a relapse by reducing risk factors; 
Improves knowledge of risk factors; 
Prepares people to go back to work.
