Description of service(s) | Providing information and confidential consultation to families about the nature and course of HD, techniques of care and research progress. Providing information and consultation to community agency staff and health care professionals regarding HD, care strategies, and recent research developments. Offering short term, confidential counseling to individuals and families to help them to adjust to the ongoing changes that those affected face: including future planning, introduction to genetic testing, etc. Referrals to community resources, as necessary. In some communities,support groups for individuals with HD, those at risk, caregivers or couples are provided. Helping families to connext with community services and resources, financial assistance programs and long-term care facilities. |
